探討台灣原住民Adiponectin SNP276G>T、SNP45T>G基因多型性與中央型肥胖對血糖狀況之影響(Effect of adiponectin gene 276G>T、SNP45T>G polymorphism and Central Obesity on glycemic status in Taiwan Aborigines)

背景- adiponectin 是脂肪細胞所分泌的蛋白質具有調節胰島素抗性、肥胖和血糖平衡的能力。在研究中,我們以台灣原住民作為對象,Adiponectin 基因的多型性,SNP276G>T與SNP45T>G與高血糖之相關性,並討論中央型肥胖與Adiponectin 基因多型性交互作用對血糖的影響。

材料與方法- 本研究利用病例對照研究法。本研究共招募550人台灣南部原住民,經選擇後篩檢得88位Hyperglycemia (fasting blood glucose≧126mg/dl)及462位Control (fasting blood glucose≦100mg/dl,)。人口統計學與飲食習慣資料是由已訓練之訪員利用結構式問卷收集。SNP276G>T和SNP45T>G基因多型性是利用Taqman PCR(Taqman polymerase reaction)和聚合鏈反應(polymerase chain reaction)放大DNA片段後,再以限制片段多型性(restriction fragment length polymorphism,RFLP)方法分析受試者的基因型。

結果- 我們發現SNP276 攜帶T對偶基因的頻率其血糖在高血糖組比對照組顯著的高(0.196)(P=0.014)。SNP45T>G的頻率在高血糖組與對照組兩組之間相類似。SNP276 攜帶T對偶基因的Adiponectin 濃度與禁食血糖是呈現相反的趨勢反應(P for trend<0.05),且胰島素抗性是邊緣顯著且呈現正相關(P for trend=0.06),利用複邏輯迴歸分析,SNP276帶有T對偶基因(OR=2.02, 95% CI:1.07-3.79)和中央型肥胖(男性≧90,女性≧80)(OR=4.65,95% CI:2.00-10.78)與罹患高血糖之間是獨立相關(P<0.05)。 且SNP276G>T與中央型肥胖對罹患高血糖之危險性具有加成作用(OR=13.59,95% CI:3.64-50.78)。

結論:SNP276攜帶T對偶基因與高血糖是相關的,若中央型肥胖與SNP276攜帶T對偶基因兩者存在下,對於高血糖之危險性可能具加成作用。

Background adiponectin is an asipocyte-derived adipokine which modulate insulin sensitivity, obesity and gucose homestasis.in th present study, we analyzed the associations between two single nucleotide polymorphisms (SNP276 and SNP45 ) of adiponectin gene and glycemic status in Taiwan aborigines, and then to further examine whether presence of central obesity may modulate the impact of these polymorphisms on glycemic status.

Methods and Materials five hundred and fifty Taiwan aborigines were recuited for this case-control study. Hyperglycemia group (n=88) was defined as blood glucose ≧126 mg/dl or and those<100 mg/dl as Control group(n=462). Both groups were matched for age, sex and tribal background. Demographic data and dietary habites were collected by trained interviewers using structures quantionaires. Taqman polymerase reaction and polymerase chain reaction – restriction fragment length were use to analyses the genotype.

Result We found that frequencies of SNP 276 T allele(0.278) were significantly higher in hyperglycemia than those in Control subjects (0.196)(P=0.014). The frequencies of the SNP 45T>G were, however, similar between hyperglycemia and Control subjects. Fasting glucose were positive related to dose of SNP 276T allele (P for trend <0.05), while adiponectin was invesery corrlated and insulin resistance was marginally and positively correlated (P for trend=0.06). In a multivariate adjusted logistic model, SNP 276T containing genotypes (OR=2.02, 95% CI:1.07-3.79) and central obesity (OR=4.65 ,95% CI:2.00-10.78) were independently related to hyperglycemia. We futher found that SNP 276T containing genotype and presence of central obesity were synergistically associated with greater risks of hyperglycemia (OR=13.59, 95% CI:3.64-50.78).



Conclusions SNP276T variants is associated hyperglycemic status and presence of central obesity and SNP 276T variant may exercise synergistic effects on risks of hyperglycemia.
