在台灣南部某原住民族群GHSR基因多型性與代謝相關數量性狀之關係(The association of GHSR gene polymorphisms with metabolism-related quantitative traits in an aboriginal tribe in southern Taiwan)

[ 摘要 ]

研究目標:探討台灣南部地區某排灣族原住民族群生長激素分泌促進劑受體growth hormone secretagogue receptor (GHSR,Ghrelin receptor)單一核苷酸多型性與代謝相關危險因子(腹部肥胖、高空腹血糖、高血壓、高三酸甘油酯、高膽固醇)及其數量性狀之關係。

研究設計:以橫斷(cross-sectional)研究方法收集台灣南部某排灣族原住民社區18~80歲原住民的問卷、血液檢體、人體測量資料及血壓,用於分析GHSR基因5’端鄰近區域-1058 A/T、exon1 171T/C、477G/A單一核苷酸多型性對代謝相關危險因子及其數量性狀的影響。另將全部研究對象分為成人組(18~64歲)及老年組(65~80歲),並個別分析。

結果:在全部排灣族原住民中,經校正性別、年齡及BMI值後,-1058A/T SNP與舒張壓顯著相關(P=0.032),171T/C及477G/A SNPs與空腹血糖(P=0.038、P=0.012)顯著相關。GHSR -1058A/T、171T/C、477G/A SNPs與腹部肥胖、高空腹血糖、高血壓、高三酸甘油酯、高膽固醇沒有關聯性。

在18~64歲成人組中,經校正性別、年齡及BMI後,-1058A/T SNP與平均動脈壓(P=0.046)、總膽固醇(P=0.007)顯著相關,171T/C、477G/A SNPs與總膽固醇(P=0.001、P=0.015)顯著相關。GHSR -1058A/T、171T/C、477G/A SNPs與高膽固醇有關聯性存在(P<0.05),而且GHSR單一核苷酸多型性的對偶基因型之間或單倍型之間都存在顯著高膽固醇危險對比值。然而在65~80歲老年組中,GHSR –1058A/T、171T/C、477G/A SNPs與代謝相關數量性狀皆沒有顯著相關。

結論:在本研究之18~64歲排灣族原住民中,GHSR –1058A/T、171T/C、477G/A單一核苷酸多型性與總膽固醇濃度都有顯著相關,並且與高膽固醇有顯著關聯性。GHSR單一核苷酸多型性可能影響18~64歲排灣族原住民總膽固醇濃度。

Objective: The objective of this study was to examine the relation of ghrelin receptor (GHSR) single nucleotide polymorphism to metabolism-related risk factors(central obesity, high fasting glucose, high blood pressure, high triglycerides, and high cholesterol) and their quantitative traits in one Paiwan aboriginal tribe in southern Taiwan.



Design: In order to analyze the effects of GHSR gene 5’flanking region –1058A/T, and exon1 171T/C, 477G/A SNPs to metabolism-related risk factors and their quantitative traits, we conducted a cross-sectional study, then collected the questionnaires, blood samples, anthropometric data, and blood pressures of aborigines between eighteen and eighty years old in a Paiwan aboriginal community in southern Taiwan. Additionally, all study subjects were divided into adult group(18~64 year-old) and eldly group(65~80 year-old),and were analyzed separately.



Results: After adjusting for sex, age, and BMI, -1058A/T SNP was significantly associated with diastolic blood pressure(P=0.032) in all Paiwan aborigines ; and further 171T/C and 477G/A SNPs were significantly associated with fasting glucose(P=0.038,P=0.012). GHSR -1058A/T, 171T/C, and 477G/A SNPs was not correlated with central obesity, high fasting glucose, high blood pressure, high triglycerides, and high cholesterol.

In adult group between 18 and 64 years old, -1058A/T SNP was significantly associated with mean arterial pressure (P=0.046) and total cholesterol(P=0.007) after adjusting for sex, age, and BMI; and further 171T/C and 477G/A SNPs were significantly associated with total cholesterol(P=0.001,P=0.015). -1058A/T, 171C/T, and 477A/G SNPs were correlated with high cholesterol(P<0.05); moreover there were significant odds ratios of high cholesterol either between the alleles of GHSR SNPs.or between the haplotypes of GHSR SNPs. However, in eldly group between 65 and 80 years old, GHSR.-1058A/T, 171T/C, and 477G/A SNPs were not associated with metabolism-related quantitative traits.



Conclusion: In Paiwan aborigines between 18 and 64 years old of this study, GHSR -1058A/T, 171T/C, and 477G/A SNPs were significantly associated with total cholesterol level and significantly correlated with high cholesterol. GHSR SNPs may influence total cholesterol level of Paiwan aborigines between 18 and 64 years old.
