集集地震6個月後邵族人精神疾病的流行病學調查(The Epidemiological Study of Mental Disorder among Thao Tribe 6 Months after the Chi-Chi Earthquake)

[ 摘要 ]

研究目的: 探討九二一集集地震6個月後,聚居日月潭畔之邵族人的各種精神疾病盛行率和相關因子,並針對這些精神疾病予以心理重建治療與追蹤。

研究方法:本橫斷性研究從公元2000年3月底到同年4月初由受過精神科專業訓練二週的訪視員,先收集16歲以上邵族人包括受災情形等基本資料後,再由兩位精神科專科醫師以中文版迷你國際神經精神疾病會談量表(MINI)結構式訪談,確定各種精神疾病的盛行率後,轉介給予負責魚池鄉緊急精神醫療之機構開始定期治療與追蹤。共完成146人。訪視率為67.3%。

研究結果:研究結果發現在146名16歲以上邵族人中,52位有酒精依賴或濫用疾患,盛行率為35.6%;32位有睡眠障礙,盛行率為21.9%;29位有重度憂慮症,盛行率為19.9%;20位有創傷後壓力,盛行率為13.7%。邏輯迴歸分析結果發現,集集地震6個月後,16歲以上邵族人的創傷後壓力症候群的顯著相關因子: 損失三十萬台幣以上 (yes vs no, OR=17.8, 95% CI: 2.3-136.3), 地震中親友受傷 (yes vs no, OR=2.3, 95% CI: 1.4-3.8), 破裂的婚姻 (yes vs no, OR=3.3, 95% CI: 1.3-8.0), 地震後就醫 (yes vs no, OR=5.5, 95% CI: 1.2-24.9), 女性 (female vs male, OR=3.2, 95% CI: 1.1-9.3), 地震中自己受傷 (yes vs no, OR=3.3, 95% CI: 1.01-10.7)。

結論:本研究可以發現大地震6個月後16歲以上邵族人各種精神疾病盛行率,並且找出創傷後壓力症候群和重度憂鬱症的個案做持續性的追蹤治療,以提供其較為完善的精神醫療服務,並據以做為精神衛生政策預防保健的參考依據。

Objective: To study the prevalence and associated factors of psychiatric morbidity among Thao tribe 6 months after a nature disaster in Sun-Moon Lake, Taiwan.

Methods: A cross-sectional study on psychiatric morbidity among Thao tribe subjects, screened by well-trained interviewers for inhabitants aged 16 and over in Sun-Moon Lake from March 2000, was carried out on April 2000 by two psychiatrist using the Mini-international Neuropsychiatric Interview (MINI). A total of 146 (67.3%) had been examined.

Results: Fifty-two(35.6%) of the subjects had been diagnosed with alcohol dependence or abuse ; thirty-two(21.9%) of the subjects had been diagnosed with sleep disorder ; twenty-night(19.9%) of the subjects had been diagnosed with major depression ; twenty(13.7%) of the subjects had been diagnosed with posttraumatic stress disorder . Based on multiple logistic regression analysis, the significant associated factors for posttraumatic stress disorder were property- loss more than 300.000 NT dollars (yes vs no, OR=17.8, 95% CI: 2.3-136.3), relatives injured (yes vs no, OR=2.3, 95% CI: 1.4-3.8), broken marriage (yes vs no, OR=3.3, 95% CI: 1.3-8.0), seeking medical help (yes vs no, OR=5.5, 95% CI: 1.2-24.9), sex (female vs male, OR=3.2, 95% CI: 1.1-9.3), and self-injury (yes vs no, OR=3.3, 95% CI: 1.01-10.7).

Conclusion: This study found that the prevalence of major depression and posttraumatic stress disorder among Thao tribe 6 months after Chi-Chi earthquake were 19.9% and 13.7%. The attention to the mental health needs of disaster survivors is indicated.
